Job Description

Assess the needs of those with acute, complex or enduring Mental Health problems and the needs of their carers.

Together with the service users and carers, formulate and implement care plans appropriate to their needs.

Monitor, review and evaluate services in order to ensure that they are provided effectively and in accordance with need.

Development and implementation of programme activities in partnership with members (service users).

Assessment of needs of members and their carers, and coordination of community support for members.

Development and management of relationships with employers as part of vocational rehabilitation programme.

Development of broad community networks that support the social inclusion of members.

Monitor, review and evaluate services in order to ensure that they are provided effectively and in accordance with need.

In partnership with members, develop and implement activities that will create services and resources that will be of value to members. Such activities might include supporting members in competitive employment, running a cafeteria, producing a newsletter, maintaining a database of employment records or organizing outreach calls to members. Staff are expected to facilitate multiple activities at the same time, while maximizing members’ opportunities to take responsibility for those activities.

Develop and manage relationships with employers as part of a vocational rehabilitation programme. This includes creating and supporting ‘Transitional Employment Placements’, in which the programme takes on part-time positions that members fill for 6-9 months each, with staff responsible for facilitating the selection and training of the member, and providing a guarantee of coverage if the member is absent or cannot maintain full productivity.

Assist members to identify and achieve their goals and to gain access to medical, housing, benefits, employment and education resources. For some members, be responsible for implementing the Care Programme Approach, and for others. to attend and actively contribute to the CPA process.

In partnership with members, access community opportunities and develop and implement activities that build members’ social networks.

In partnership with members, access educational opportunities and develop and implement educational activities that meet members’ employment and other educational needs.

Take an active role in fundraising and public relations initiatives together with members.

Participate in quality assurance measures such as Clubhouse accreditation, peer review, Charter Mark award and members satisfaction surveys.

To ensure necessary record keeping is maintained in partnership with members.

To participate in regular supervision sessions with the programme manager.

Take on additional responsibilities such as Fire officer, Health and Safety Officer, First Aid, Food Hygiene, COSH, etc.

In partnership with members, keep up to date with best practice in mental health provision, government initiatives and guidelines and other models of rehabilitation.

Attend a 3 week training and conferences within the first year, in the UK or abroad, with members. Undertake other training as required.

Provide an assessment and care management service to individuals, their carers and other agencies.

Initiate and co-ordinate care plans for and with individual service users and their carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.

Through the assessment and prioritisation of need, purchase appropriate services within budget provision.

Monitor the efficiency and effectiveness of individual care plans.

Review the needs of individual service users and their carers.

Identify and report unmet need and service short falls in the construction of individual care plans.

Maintain client records in accordance with policy.

Respond to and where possible resolve complaints within the Council’s complaints procedure.

Contribute to service development by providing information and ideas for future services.

Advise manager on quality and value for money of purchased services.

Co-ordinate, participate and where appropriate chair case conferences/planning meetings.

Participate in multi-disciplinary and inter-agency meetings and working groups and represent the service as required.

After training, perform the duties of an Approved Social Worker.

Any other duties as determined and required.
